Upgrade Notes
Update error handling for the renamed `creationFailed` to `failed` lifecycle state and switch to chat-based forking. Remove the `error` field from `changeset/contentChanged` and handle `changeset/statusChanged` failures instead. Update error code handling as `ContentNotFound` is no longer exported.
