Upgrade Notes

Do not mix 4.x and 6.0 nodes in a replication set; upgrade all nodes together. The EFFECTS_THRESHOLD config parameter is accepted but ignored in v6.0. Use TIMEOUT_DEFAULT and TIMEOUT_MAX instead of the deprecated TIMEOUT.
