Upgrade Notes

Upgrade every node in a replication group together as 6.0 uses a new replication format v3 that 4.x cannot read. The `EFFECTS_THRESHOLD` config is accepted but ignored; use `TIMEOUT_DEFAULT` and `TIMEOUT_MAX` instead of deprecated `TIMEOUT`.
