Upgrade Notes

Update code to use `buildPreview()` and `PREVIEW_MAX_LENGTH` as `buildInputPreview()` is deprecated. Remove any usage of Plan/Explore modes in Factory sessions or update configuration to use the new global thinking setting. Ensure custom credential providers explicitly define `credentialScope` as cross-scope fallback is removed.
