Upgrade Notes

To use mosh, install `mosh` locally and `mosh-server` on the remote, then configure `transport = "mosh"` and `mosh_server` in config. Ensure your telemetry configuration is compatible with v1.16.18's endpoint change if you are upgrading from an earlier version. Note that Claude default model setting is now honored and Codex accepts `--effort max` and `--effort ultra`.
